Title: Effective Patient Engagement Strategies to Improve Diabetes and Health Equity Goals in Large Healthcare Systems

These issues will be placed in the context of strategies that leverage community-based partnerships, encourage sustainability and dissemination, and seek to reduce disparities within vulnerable populations. The talk will also discuss health services research and policy approaches to measuring diabetes prevention efforts and disparities reduction that can learn from existing disease prevention metrics and reporting models.
